UPCAT 2011 application form
The application form can be obtained in person against a crossed bank demand draft of Rs. 1000/- (Rs. One Thousand Only) (Not by cash) drawn in favour of The Association of Private Medical & Dental Colleges of Uttar Pradesh payable at Ghaziabad.
The application form must reach before the last dates of Receipt date of the application form.
