Application Form can be obtained from Branches of Oriental Bank of Commerce by cash payment of Rs.1000/-from Co-ordinator UPCMET, 1st Floor, Anand Ashram Complex, Rampur Garden, Bareilly – U.P. – 243 001 the application formal also obtained by sending Bank Draft of Rs.1050/- in favour of “UPUMCWA, Bareilly” along with two self-addressed stickers,   April, 2010 and iii) from the member institutions on cash payment of Rs.1000/-.
